Let me guess: you screwed in a bit too tight, and the eyelet washer bent and popped off? Me too. haha

Actually, ACE Hardware store has them in different sizes. If you have an ACE by you take one in and match it up. I replaced all of mine on an Bogner 2x12 I had early this year.
