Flat right angle 1/4" jacks.

Anyone know where I can get the real flat right angle jacks? I'm getting ready to rack all my pedals. Also, where can I get a good sliding rack shelf? Any help would be greatly appreciated!

The original design is Switchcraft model 228. I have bunch of them for various applications. they are low profile and almost indestructible but not too easy to find (try at ebay)
