I not sure Brian Thompson believed he was doing anything wrong. The protocols were written and he was just doing what the job demanded of him. I cannot imagine waking up and thinking; how much money can I make by fucking other people. That’s just a weird mindset. On the other hand, the shysters of the world are actually scheming how to rip people off. 2 bit hustlers, gamblers, porch pirates etc; Always looking for another dime with “plans and ideas”. Don’t get me wrong I’m not for or against the insurance company. I just know how they think and act; justify and prove why you want money for the treatment and they say ok. It’s quite cut and dry. If the insurance company says no; chances are the boxes were not checked correctly or there is no indication for treatment. It’s very bureaucratic. So in my practice, I make sure I know the rules then follow them and people get taken care of.CEO of United Healthcare learned that lesson.

CMS center for medical services (govt agency) sets the rules and guidelines and all insurance companies have to follow. Or they can get in trouble. I agree the natural answer is NO from the insurance provider. That’s why all boxes have to be checked and you have to go by the criteria mandated by the government . Which are made up by “science” (bad word now) or scientific data to support say “high grade coronary blockage stent, hip replacement etc”. I don’t know that I would go as far as comparing nazi-isms to healthcare. Idk. Maybe I’m in a bubble because I see things from a different viewpoint. I know how to check the boxes to get things done. I know what they are looking for so it’s easy for me. BUT I can see how a lot of folks not in the know would get pissed and frustrated of why it’s not working for them including healthcare providers. Having worked in IT for a few health insurance companies early in my career, I am sure they have data, history and statistics; if they wanted, the can use that information to show correlation between denied procedures; success rates of procedures; impact of procedures on survival durations; avg survival post-procedures; additional, related, alternative procedures; date/cause of death, etc.
CEO sets the goals and objectives; those goals and objectives are distributed across, down and throughout the organization; senior decision makers determine the strategies that will be used to meet those goals and objectives, and management layers down to the lowest level are either told the tactical requirements that will be used to achieve those goals or allow some levels of middle management to make those interpretations, or push them down the line...I've seen both where decisions are made 3-4 levels up, and pushed down to the lower levels who are glorified messengers; or, the goals, objectives, expectations and constraints are provided to the lowest level manager who determines the tactics that will be used.
Ultimately, the CEO sets the tone and marching orders for the company; few companies allow autonomy at the lowest levels, especially if they're highly regulated like insurance and other financial services.
